Transitional housing is provided at a triplex in Observation Hill in Duluth, MN. This is a program that accepts individuals by referral from SOAR Career Solutions. Residents work with SOAR, are expected to obtain and maintain employment, follow conditions of parole/probation, attend weekly Health Realization meetings, and fulfill other expectations. Each resident has their own room. Currently the program is only for men and it is expected that residents will be drug and alcohol free.
Health Realization is a cognitive skills-based program that helps individuals manage stress and work through barriers. It is often considered an addiction recovery program or support group, and while it is effective for people with addictions, it can assist with a wide range of life issues, such as stress and anxiety. It considers substance abuse to be a lack of self-efficacy and aims to help individuals understand their own role in creating stress. It focuses on present needs and thoughts rather than past errors and problems. Health Realization assists participants in responsible decision-making and self-management, supporting their successful re-entry.
Enroll with SOAR Career Solutions to get started. You will work with a Re-Entry Case Manager throughout your time with Vita Nova, and they will help you access the Vita Nova application and other processes.
